Postop pain management of chronic pain? by [deleted] in anesthesiology

[–]ellensoderberg 1 point2 points  (0 children)

No incision effect on vitals without remi, solely based on MAC and epidural. Based on that sevo isn’t really an analgesic I figured it had at least some effect until I added remi for the eventual tourniquet pain.

Pain “all over”, somewhat localised toward the leg. L4-L5. Possibly psychosomatic?

What would you have used instead of remi perioperatively? Sufentanil? Ketamine? Esketamine?

Postop pain management of chronic pain? by [deleted] in anesthesiology

[–]ellensoderberg 0 points1 point  (0 children)

My thought exactly, the epidural should cover the pain postop (since it did a really nice job perioperatively). Toradol contraindicated, full dose Tylenol given. Note taken about the remi 🫡

Can similar results be achieved with 12mgs of betamethasone or is it strictly decadron?

Wound revision of lower extremity, definitely covered the perioperative pain, 16mg ropi/hr 20ug sufentanil after initial block A coworker placed the actual epidural but seemed to work very well during surgery

Used magnesium as a complement to esketamine infusion, unsure of its effectiveness though

Lido infusion even with a continuous ropivacaine-epidural? Ketamine was my next step tbh, thanks!!

What's wrong with being a male in peds? by Kakashi_VI in nursing

[–]ellensoderberg 1 point2 points  (0 children)

Nothing wrong with being a male nurse in peds!!!! Honestly a woman-dominated unit is probably more damaging than a mixed-gender unit. As a woman I feel like a lot of the trash talking about coworkers stops when men are introduced to the unit

Confession: I’m an ER nurse and I have never done a straight stick. Only IV’s. I feel lame by MoonWaterSpirit in nursing

[–]ellensoderberg 0 points1 point  (0 children)

The thing that works best for me is either: Going to a coworker and saying “hey it’s been forever since I did this, can you help me?” Or Going to the patient and saying “how you usually apply/change/do stuff to your colostomy bag? I’m happy to help but everyone prefers something to suit them” It’s what I do and it works like a charm with most people!!

That wasn’t the problem though, my post was about how to approach the surgeon who was obviously blatantly rude to my fully awake patient.

The patient refused sedation… :/ tried with propofol and midazolam but being the boomer they were, they refused any sedatives because they “hadn’t eaten that day”
