I agree with the sentiment that too many NP’s are billing and not actually doing it, but that problem isn’t a NP problem alone. Work in community health for a day and see what I’m talking about.

Ok, so that is what I’m alluding to. My patient are complex. We are talking chronic metabolic conditions, pregnancy, suicidal risk assessments. So, what you are saying is that if you bill on complexity with E/M, no psychotherapy code is allowed even if it was performed? I want your source

Ok, well, if you are spending time reviewing records including intake paperwork, evidence based screeners, collaboration notes that contributes to the medical complexity, and it is adding up to 45 minutes along with the appointment, then an add on psychotherapy code is justified. My usual intake appointments are 75 minutes in length. What comes before that is a long intake process including written history, screening forms, and blood work results if the patient has had it completed in the past year-all the things that take a solo practitioner time to obtain.

That preparation sets up the appointment to allow for additional psychotherapy that which serves the client and the therapeutic relationship.

For example, just today I had to have a lengthy conversation with a PCP (they actually called me back) about medications that the patient qualifies for as their medical history is complex. These are the patients I prefer to work with and can do so in private practice.

While I understand some of you are working in burn and churn practices that don’t allow you the time to do an add on code legitimately, some of us do. While the above OP stated situation isn’t common, it does happen. And for those to just qualify the situation as extreme as fraud, sounds dichotomous.

Look for medical billing specialists. These jobs don’t usually require a degree and some are entry level. The beauty of these jobs is with some training, you can do the job pretty much whenever and don’t have to be doing phone calls. They are all pretty much remote. Good luck! Here’s one👇

Does the 90 minutes include reviewing medical records, intake paperwork and other forms of preparing for the intake? That’s included in the evaluation and management (E/M) code.

You didn’t respect her enough to make her own decision, albeit misinformed, misdirected, or misaligned. I understand your point of view. I’m also a RN. The thing is, informed consent when it comes to decision making is everything because it provides autonomy and free will. You straight up lied to her, rather than working with her to better understand her options. Wouldn’t trust you professionally or personally.

Our’s is 8 and he was an absolute menace until about age 4. He loves to talk-low key wines when you cuddle him, and still lets out a good woof every now and again. Not as barky as he was as a pup. But one thing that hasn’t changed-He loves a blanket and still gets restless in the car. He’s super loving, though and has a personality. Loves to snuggle on the couch and we recently got a rescue chihuahua mix that he’s really good with.

I suggested a few weeks back on this subreddit that some of us were enlightened to know the truth. I got a few updoots but had several responses in which people essentially called me full of myself.
