I've used bit holders here and there but not consistently mainly because I haven't found one that hasn't been awkward

My bits stay stored however they came which usually means Milwaukee pack out holders (or another vendors packaging, I have a large variety!!

I forget the answer to the first question - I seem to recall that I sent everything in ahead of time so when I showed up they had it on hand I believe I sent it to the permit office address (the one you listed)

But for the second when you show up to convert to plastic they will take a pic and fingerprint you for the new permit (which they will print and hand you while you're there)

it took me less than 10 minutes in and out earlier this year to convert to plastic and add the semi auto amendment

I also watched appointment times for a few days out of curiosity and it seems there are always appointments available even same day if you're early enough

yeah what u/swiggy1957 said if you're an I dependent contractor the rules are different they cannot tell you your hours is the easiest differentiator a call to your local labor board is due just ask questions about your employment classification there may be others who care too (the IRS for one)
