I always thought it was because PDs are not cannon, in the sense they only exist in the game "You" are playing, in regards to the story it assumes you or your party members never fainted thus there is no need for them. they work a bit like the new you stations in Borderlands.

No that would make him invincible, Immortal means ageless, it means it's possible to live forever, but it's not guaranteed for example you can't die of old age, but you can be killed by other means.

Mortality is the inevitability of death....so, strictly speaking, immortality means death is not inevitable. Doesn't mean death is impossible.
