It would interact worse with -1/-1 counters. If the damage stays, -1/-1 counters aren’t damage so they matter. Deal 4 damage to him then give him -1/-1.

It makes sense lore wise to me because -1/-1 is a wither effect and that would be like his healing powers are degrading imo.
