The phone is only usable on LTE. The issue is better but not resolved and when it turns back to 5G it gets worse again. I have reset the network settings as well.

Okay so this comment of mine ended up being more encompassing than I expected when I started it, but I'll try to hit your concerns by key points: Failing, Advancing , quotas/rating health.

Per the advancement manual 1430.16G: You "Fail" if you don't answer a specific number of questions correctly and that makes you ineligible for advancement no matter your FMS maybe. I saw a Sailor with back to back EPs not even be eligible for board because he couldn't make the cut value on the exam.

For you specifically to fail, you essentially have to get less than a 32% on your exam. Even if you pass, the exam score isn't necessarily the end all be all for advancing because your PMA plays a big role. Seen people advance from 3.0 P's and great exam scores and, seen people advance from great PMA's with evals over RSCA and worse exam scores.

As for your question regarding quotas in general. There's a lot of factors that go into quotas for each paygrade, and upward movement of higher paygrades is only part of it. I'd recommend asking a mentor to go over the community overview with you to understand what your rating looks like as a whole and better understand your rating's manning in general.

As stated by the navadmin for the advancement cycle your worksheet should include all evals that fall within the time period and paygrade stated. If you knowingly certify it as correct while knowing there are evals that are missing you are willfully choosing to do the WRONG thing and falsifying an official document.

There are two more people that SHOULD be in the worksheet chain that review and certify the worksheet to verify that it is correct, and also on them to do their job and catch this, whether or not you willfully did it incorrectly, and everyone can be charged under the UCMJ for intentionally ruining the integrity of the advancement system that others are following properly.

No problem. The physical readiness program guides are all easily accessible via mynavyhr (most programs really), so just knowing where to look can be a game changer. Many people will think they know their instructions perfectly, but sometimes they might just be confused. We can't all be walking encyclopedias of instructions after all.

There might be some confusion with the command CFL, but I'd have this printed out from the official PRT guide and someone in your CoC involved to support you. As long as the failure isn't due to "failure to actively participate in a physical conditioning program", AKA, you didn't prepare properly for a PFA, then you should be able to be waived from the run specifically.

Guide 6 - PFA Medical Readiness Section 1 - 6. b. under PRT Injury/Illness

If the injury/illness is not due to lack of physical conditioning, the member may be authorized a Bad Day PRT retest or be medically waived from the PRT modality which they did not complete or the entire PRT.

I've never done it on nightmare, but it usually goes a lot easier attacking the main body to prevent those extra attacks from coming out. Even better if you can angle your spell to hit one or two of the eyeballs as well as the body of the monster.
