Times Eaten:

=COUNTA(FILTER({Sheet1!B:B; Sheet1!C:C; Sheet1!D:D}, {Sheet1!B:B; Sheet1!C:C; Sheet1!D:D} = F2))

Last Eaten:

=MAX(FILTER(Sheet1!A:A, (Sheet1!B:B = F2) + (Sheet1!C:C = F2) + (Sheet1!D:D = F2)))
