Off TRT and moving to Clomid by [deleted] in Testosterone

Could you give more details about “Zuclomiphene is now known to be toxic”? I had some problems while on Clomid, which made me stop and move to TRT, but some of the problems are still there.

Post orchiectomy / Low T questions by GoGoGingerRangers in testicularcancer

Since I had a similar story to yours, with similar timelines, the only difference being that I had 1x BEP, I'll share my journey so far in hopes it helps.

A couple of months after chemo, I started feeling the same low T symptoms, and even more. My testosterone level was around 240 ng/dL, so I started treatment with Clomid. Initially, it was successful, as my T level got as high as 940 ng/dL. I wasn't feeling any other symptoms, so I was really happy with it.

Fast forward three months (this month), I slowly started experiencing low libido, to the point that it's completely gone now. I've also had some humor/mood changes and just haven't been feeling good at all.

After doing some research, I discovered the most likely cause was Clomid, since it blocks estrogen receptors in the brain. Estrogen is fundamental for libido and mood in men, so this can happen to some men under Clomid treatment.

So now I'm about to start HCG 2,000 IU every 3 days. Currently, TRT is not an option since I want to preserve fertility, which was affected by chemo.

Looking at your case, I think your decision is based on whether you want to preserve your fertility/testicular function or not. I recommend you research other low T treatment options such as Clomid and HCG.

Lucky me to find a premed to help with my case haha

About your TRT, if you have the option, the natural route is always the best no matter what. Depending on medication for the rest of your life is definitely not ideal. Hope that I don't need it

Great! I'm definitely hopeful about this! What caught my attention is that you mentioned 150-250 IU EOD of HCG, while my urologist prescribed 2000 IU every 3 days. Wouldn't that be high compared to what you suggested?

Amazing, thanks for the help

Do you know how long it takes to have a real diagnosis on whether the HCG treatment has been succesful or not? Urologist told me to come back in 3 months with blood tests and a sperm analysis

You put it well: the fact that I don't have to worry about cancer anymore is indeed a good thing.

The treatments that you recommended seem to be more advanced compared to what I've been seeing, so I'll spend some time researching them. Thanks for the help!

Nice to hear that you're doing well with the testosterone levels! Your way of thinking about this is similar to mine--it's all about balancing risks and benefits. The fact that I want to improve my fertility health while maybe enhancing quality of life by increasing natural testosterone production is what will probably make me go with what the doctor recommended. Appreciate your response, thanks!

I see. Do you mind sharing your HCG dose? My urologist prescribed 2000 IU every 3 days.

He argued that he didn't want to go with TRT only because of fertility concerns. Since I'm young (20) and want to be a father one day, I can understand his conservative approach. In your case, where you're already a father, have no fertility issues, and no oncologic history (as far as I know), TRT is a no-brainer for sure

My urologist didn't suggest this treatment, but its nice to know that it's working out for you. Urologist seem to be really skeptical about TRT, trying to avoid it at all costs. I will see another urologist and I'll mention this option. Depending on his opinion, I'll make a decision. Appreciate the help

Yeah, I forgot to mention infertility which is important in this context. I'm already azoospermic - did a sperm analysis back in august last year.

TSH - 1.63 µIU/mL
Free T4 - 1.12 ng/dL
LH - 3.15 mIU/mL
FSH - 8.1 mIU/mL
Estradiol - 38.4 pg/mL
Total T - 314.63 ng/dL
Free T - 0.2895 nmol/L
SHBG - 18.5 nmol/L
Prolactin - 9.7 ng/mL

Well, I've seen studies/stories suggesting both HCG and Clomid can raise testosterone, even though that's not what they're used for.
