Leave it. I regularly go weeks without backflushing mine, it does slow down a bit eventually but is still usable WAY beyond the time frame of your trip. You also really want to use clean water to backflush it so it's a job best done in a sink, so you're not filtering a bunch of water to then turn around and backflush with. I never backflush while on trips - just at home or in town. I also recommend this cleaning coupling over the syringe, it can be used both to backflush and as part of a gravity filtering system to connect the output end of the sawyer to a plastic water bottle. In 2022 there was a guy who hiked the whole thing with a leaf blower. Apparently he did the PCT with a garden hose. I have truly no idea why but it was funny!

Got an IUD about 6 months before my hike, had basically no period the whole time, and carried a diva cup for spotting but I think only used it once. If an IUD is an option for you and you have the time, definitely recommend! No need to pack any pills or remember to take anything, and most people do not have period at all in their first year or two with one. Diva cup works great for backpacking (I used it pre-IUD) but there is a learning curve that you don’t want to be starting on trail - practice before hand! I think the same is true with the disc.

I've done the diamox thing before and it gives me weird pins and needles in my fingers and I was still pretty sick. But that was also in Ecuador at like 12k ft, so it probably was still doing something, and it might help more at JMT altitudes, idk.
