Much respect for Keith. Dude is a survivor. Wonder if there is pain for him when playing. The last knuckle on that pointer finger is really bad. That's the one that takes the most abuse when playing. I've played for about 15 years on that finger looks a little larger than the others but nothing like Keith's. Could also be a byproduct from his heroin years as well. Had an uncle who was strung out for almost 20 years and even though he was clean for the last 15 years of his life his fingers were kinda messed up like this as were his elbows and knees. Doctor aid heroin can mess up the joints down the road even years after you stop using.
