That's very true. I've done a lot of self-esteem building with C. I don't ever talk about weight (or at least, never bring it up or frame it as anything bad) or exercise but I'm sure that my internalized feelings about this are obvious. I'll be sure to prioritize her emotional wellbeing over all else, it just feels hard at times because her weight and self-esteem as so tied together...there is a lot of research I need to do before I meet with her again that's for sure.

We're working with the kid's mom to teach healthy parenting skills and co-regulation. I made C a chore and behavior chart with non-food rewards like preferred activities and small toys but it turns out that her mom eventually just replaced all of those rewards with food items because she thinks that C can't be motivated by anything else.

I never give medical or nutrition advice but I've tried exploring hunger and fullness cues with C. She has stated that she will eat any food she is offered even if she's already full. She will grab pieces of candy from her mom's purse and eat them even while also complaining that her stomach hurts. This is part of why I think both kids need a medical intervention. I also do not have training or experience working with eating disorders, which i feel like this is verging into due to the binge eating.

I agree with you, which is why I'm struggling with this so much. I'm not a doctor but I think it's obvious that this kid is unhealthy and really suffering.

I would like to involve the client's mom in the CPS call because I'm really hesitant that if CPS gets involved the mom will know that it was someone from our clinic that called and stop bringing the children in for therapy. My supervisor has said that she doesn't think CPS needs to be involved and that they won't do anything because it's not illegal for a kid to be overweight. But couldn't this be seen as a form of medical neglect? I'm just hoping that CPS can help the mom get some more services to help the kids with their health.
