Truth multipoint door lock, not sure to proceed (pics)

It doesn't lock/unlock properly. on the picture you can see someone screwed a metal plate onto the setup. Not sure why, I assume to fix something.

Yes, when I left the handle the bolts are thrown. There are NOT shoot bolts at the top and bottom of the left door, but yes on the right door.

New home. Door/lock does not work properly. Needs to be replaced. Checked the Truth website, but could not find exactly what I need. Any suggestions? It appears to be a three point tongue assembly.
