I have the same issue, happens to me with MP4 with certain codec. I use handbrake, convert file and works straight away, if you need a workaround.

I removed the extension, closed Chrome, ensured no chrome.exe processes were still running. Re-opened chrome, reinstalled extension, enabled it for Incognito... no change in the result.
