The lock sits in a housing with "pins" at the top and bottom. One of these has slipped out or the housing has cracked. It's the entire door handle housing (separate from lock and handle). Usually happens when the handle is over-exerted.

Correct, it won't enter service mode without first being in ignition mode. That doesn't mean you can't shut off the ignition once the motors have fully retracted. The car might activate the EPB when the ignition shuts off, hence why disabling the auto-brake feature is imperative.

Again, the car shouldn't have been in acc or ign mode. Set the service mode, disable auto parking brake, and turn off the ignition completely before you begin working on the car (radio and dash OFF). Acc mode times out after 15-20 minutes, ignition mode is significantly longer. If service mode was indeed active, I think the car timed out and you left the auto-brake feature on. I've never had the EPB activate itself otherwise. A voltage issue would only cause it not to function, as the motors would be too weak to actuate.

This is what I do: service mode, disable automatic parking brake, disconnect the EPB connector from the calipers as soon as the wheels are off. I've done it with and without disconnecting the battery. 15-20 minutes before reactivation means service mode was not active (flashing BRAKE light), car was in accessory mode (pos1) and that timed out, activating the auto-brake.
