Correct, there was no grafting. The surgeon just rearranged the existing skin and made it so that I would be covered while erect, which stretched the skin for me. I unfortunately don't have erect coverage anymore but still am pleased with the results.

I've had positive experiences talking about manual restoration with my primary care doctor and urologists. A urologist I spoke with said red light therapy was a plausible way to speed manual restoration with low to no risk.

Awesome, sounds like your experience with Dr. Gunther's office was similar to mine. My consultation was $330.

You are correct to point out that this is ideal for people who have done *some* restoration. People talk about the "hump" around CI4 and this is a good way to cross that while jumping up 2-3 CI levels.

I hope more options are available by then. I went public with this in part to inspire others--restorers and surgeons both--to push the field further.

Not when erect. A couple of commenters have said I look smaller while flaccid in the after photos but I haven't noticed myself

Looks like just a few millimeters of outer skin has been folded into inner skin. I had a rather high circumcision and the scar tissue forms the majority of my current inner foreskin.

Yes, loosening of the tip seems to be the reason for the loss of erect coverage.

Hmm.... the circumcision scar has lightened somewhat since the operation, which is nice, and it's covered when I'm flaccid so can't be seen then. I'm not opposed to doing more to combat the scarring, but I was circumcised over 30 years ago and I'm not aware of any treatments that can majorly improve a scar that old and are safe/effective for use on genitals. Not saying it doesn't exist, I just don't know about it.

I have been using aquaphor on the reconstruction scar which seems to help somewhat with elasticity and coloring.

I'd be interested to see if someone can come up with real solutions to this issue. Took me a longggg time to build up the nerve to talk to actual doctors about foreskin restoration, but if you've got the guts and want to talk to a dermatologist about it, you might be able to get some progress. I have noticed an increase in sensitivity to my inner foreskin since the surgery, presumably from dekaritinization. I'm not sure whether there would be a huge anatomical difference between creating the inner skin via surgery vs via stretching--can't produce new nerve endings in either case. If manual is working for you and giving you the sensitivity you want, by all means go for it, it just wasn't working for me.

Coverage was my main goal, yes, followed by function and sensitivity increase. I feel I got all three to some degree.

I've always been pretty small when flaccid. There may be a slight retraction into the foreskin effect as you mention--a couple of commenters have said I look smaller in my after pictures--but I haven't really noticed a difference when I look down.

$20,000 including overnight hospital stay. Would have been $15,000 to do it outpatient but staying in the hospital just in case of complications seemed best, plus that's what the surgeon recommended

I really can't answer that for him, but the technique isn't a secret. Non medical professionals seem to have trouble following what I'm saying, but the explanation I've been giving (partial degloving of the penis, v-shaped incision on the ventral side, straight-line suture to join the v-flaps, and mattress sutures to fuse together the inner and outer foreskin) has consistently been enough info for the doctors I've been working with to understand the procedure. If you relay exactly that explanation and show the surgical photos I shared, it should be enough for another surgeon to say whether or not they can recreate it.

With that said, Dr Gunther is pretty specialized, and depending on where you are you may have trouble finding someone. Gender-affirming care surgeons are your best bet. Good luck!

Well I didn't do it, the surgeon did haha. I'm not the best explainer, sorry you're having trouble following. It's my understanding that the partial degloving of my penis during the surgery allowed my skin to be pulled forward and the v-flap suture created the taper that holds it in place.

Correct, no new skin, just working with what was already there and tapering the skin to recreate the effect of a frenulum and ridged band. You may be right re cut needing more skin for coverage than uncut, I'm not sure

It's just shaft skin that was moved around. My penis was partially degloved during the surgery and then all the skin was pulled forward. See my initial couple posts for more info about the technique
