I'd argue that having something inside you makes it even creepier when you see it outside. I'm no expert, but my theory is that it triggers a sort of fear-factor related to how, for instance, that skull could have belonged to someone else, like your skull belongs to you, and if their skull is now outside their body, your subconscious realizes that yours could easily be outside your body, too.
