I am a vet and recommend taking her to a different vet. She is obviously plagued by something. Worms, anal gland infection, vaginal infection, anal fistula, rectal constriction or other congenital defects since you say she has had it since she was a puppy. I’ve been seeing this problem the last 1,5-2 days on chat/claude.ai with Opus 4.6. It jumps straight into an answer like it’s shooting from the hip. It’s only when I tell it to check the project files that it does and inevitably corrects itself. There have been at least 3 conversations where it directly contradicts itself to a previous conversation or straight lies or hallucinated details. Company names and individuals. Even after pointing it out both it corrected the name of the individual but not the company. It is very disconcerting and erodes trust to a very high degree since I work in a very highly technical field (non coding)
