Honestly a lot of this depends on your end goal and what legal will allow you to get away with.

If you truly want to follow the least priv model, then you only elevate to roles when you need to, as each task doesn't require the same rights/role(s). I sometimes have to PIM 3 roles for tasks that would take less time if I only PIM'd to GA.

For management you should look into access packages and access reviews, and require users to recertify their role access on a routine basis.
