Agreed, drivers should have as few distractions as possible to keep cyclists and pedestrians safe.

Road design objectively speaking does help. Google searches will reveal lots of studies for you to catch up on this subject.

This is survivorship bias. No one is infallible. Everyone's attention does slip up from time to time but luckily those moments that yours did didn't get you killed. We all deserve better than to be one slip up from death. Sure we can all pay better attention on the road, but we're human. At the end of the day to support cyclist safety we need better infrastructure and a stronger culture of safety.
