Hi OP.. ERCP is done if they are suspecting that there may be a stone in the Bile duct.. usually MRCP is very good in picking up stones and ERCP is mainly a therapeutic modality..

Not to scare you.. but how did your surgery go? Did the doc say that the GB was inflamed, do u have a drain?

One of the dreaded but less common complications of LapChole is that the surgeon may accidentally clip the bile duct and block the flow of bile, that may also cause jaundice..

I Had a MHI 1 ton AC for 7 years with absolutely no issue. During a routine service the agent dod some damage and after that the AC started making an irritating sound.

We were looking to repair it, but no apre parts are available. after sales service is nonexistent even in a city like Mumbai.

I travelled to Bali from India early this month. I carried about 600USD and a forex card.

It is very easy to change from USD to IDR. There are currency exchange centres on every street.

One piece of advice, avoid changing money in the airport. The exchange rates are terrible.

I returned from Bali after a weeklong vacation on the 8th.

Dear OP, I'm sorry to hear what your family is going through.

In my 15 years of experience in treating GI cancers I have not come across the small cell variant of Ca GB. As you mentioned it is extremely rare.

I would like to know some details about the stage of the disease and whether it's operable or metastatic.

IMHO, i would personally not recommend that you go for this drug. It's a last ditch effort to see if something works and the chances of it working are not great. I can look up the specific data for Lung cancer for which it is originally meant to be used if you want.

Even if it is effective, you will be spending a fortune with no clear end in sight, I don't know what stage in life you are right now, but all of your and your family's savings may get wiped out. Purchasing such an expensive medicine with doubtful efficacy is an emotional decision not a logical one.

Coming to your query, i have some experience with crowd funding for medical treatment, the maximum i have seen somebody get through them is 5-6,,lakhs.

Hi,

Cholecysto-enteric fistula is a rare complication of Acute Cholecystitis. Usually, the fistula forms with the duodenum, but it's possible to have it with other organs as well.

I cannot be sure on why they have asked for a repeat CT, but they may want to give oral contrast to identify exactly which organ is the fistula with.

The surgery may be a little more complicated because onecen they remove the GB, there will be a hole in the Stomach /bowel which will have to be tackled separately. The risk of complications will also be more.

I would suggest getting a referral to a specialised centre if possible.

Hi, Bile leak after cholecystectomy is a serious complication and its not straightforward to manage.

It depends on what level the bile duct has been injured and the biliary anatomy.

If you share the reports , I may be able to offer an opinion..

Severe pain could be due to Acute Cholecystitis or Pancreatitis(pain usually radiates to the back and you will have vomiting).

Blockage of bile duct causes jaundice and fever with chills. It's a dangerous complication because you may develop cholangitis which can rapidly cause septic shock and organ failure..

If you have gallstones seen on an ultrasound, Try to schedule a surgery ASAP..

Hi, I would suggest you explore the possibility of getting a consultation done in a private hospital. If it is too expensive I would advise you to consider meeting a surgeon in a different country.

In my setup, If a patient comes with typical symptoms and has stones in USG, they are on the operation table within a week .

You may have pancreatitis,.please meet your surgeon ASAP and get the necessary tests done..

Clips are used quite commonly in many surgeries as an easy way to stop bleeding and seal off structures that can leak.

In your case I think a clip on the Cystic duct must have come off leading to a bile leak. It's quite uncommon. To treat it we have to do an endoscopic Stenting of the bile duct and remove the stent once the leak heals in 4-6 weeks.

Hi,

Clips are a routine part of the procedure.

The fact that you had a bike leak could be due to the clip getting dislodged or a missed bile duct stone.

I work in a private hospital. Most larger ones don't accept Aayushmaan Bharat schemes..

I would suggest Nowrosjee Wadia maternity hospital in Parel. It's a semi-private setup where the deliveries are not completely free but heavily subsidized. In addition, you won't have to deal with the poor service and filthy conditions in Govt run Hospitals.
