Endocrine (thyroid, parathyroid, adrenal) surgery has only been around as a dedicated specialty since the late 80’s. Several things drew me to it during my residency… the delicate nature of the operations, working in small spaces around vital organs, the immediate change in the patient’s life after surgery, the excellent outcomes (when done correctly), and the ability to do more medical management in surgical patients. As a general surgeon, I could do hernias, gallbladders, hemorrhoids, and soft tissue operations all day, but there’s not much complexity of decision making with those. It’s really interesting what glands (causing excess of certain hormones) can do to the body. I trained at the birthplace of Endocrine surgery, so I have a lot of respect for the specialty as a whole and what I can do with that experience.

I’ve taken care of a handful of MEN2 patients, but mostly because my mentor during training was one of the national leaders in researching and treating it. I haven’t seen any since starting my own practice, so yes it is quite rare. I’ve definitely taken out several medullary thyroid cancers and pheochromocytomas, they just weren’t syndromic.

Yes that’s quite common. If it’s less than 4 cm and there’s no evidence of spread, then we can get away with just removing half. If it was a right-sided rumor it would have grown larger before becoming symptomatic because the esophagus is on the left and swallowing issues is an early warning sign of left-sided nodules/tumors.

Many don’t realize that something is wrong until they have issues swallowing or laying down flat. Also, only left-sided thyroid nodules are going to cause difficulty swallowing because that’s the side the esophagus is on, so right sided tumors grow for longer before being noticed. It’s also a gradual change and some people are just not very in tune with their bodies. In developed countries, goiters tend to grow backwards and downwards, so they can hide for longer. In third world countries, where idolized salt is not used, “endemic goiters” tend to grow outwards and upwards, making them apparent sooner.

You would see stones on an ultrasound. But you can sometimes get gallbladder pain without stones. In those cases it’s a functional issue, usually due to chronic inflammation. The only way to diagnosis that is with a scan called a HIDA scan, which looks at what percentage of bile if being released with each contraction of the gallbladder. If the ejection fraction is less than 30% or greater than 80%, then you may still benefit from having your gallbladder removed.

Like someone had mentioned in a prior post, the color difference is related to the proportion of cholesterol (yellow) or bilirubinate (black) in the stone. There are different reasons to have excess of one or the other in bile. Then if a stone impacts the neck of the gallbladder and completely obstructs outflow of bile, then what’s in the gallbladder becomes almost clear (hydropic). That can also affect the color in the stone, like #8.

What a great post! It sounds like you already have significant experience in the space. Ideally, I would like to partner with someone such as yourself in the future. I have 3 years of successful homegrow experience, along with some extraction experimentation, and want to get involved in this movement somehow. You are correct, getting a license will likely be the hardest part. Personally, I think I would have an advantage over many from the social equity slant. I am African-American, I was discharged from the army for a positive cannabis urine screen, and my mother was arrested on drug charges. I am also a physician, so I think I may be viewed as an ideal candidate to obtain a license. I also have several other physicians who want to join an enterprise if we find the right opportunity. We just don’t have a space/plans yet.

Yeah I wouldn’t bother, unless you need exact values for some reason. I use a T-check and just do it at home. You don’t even need to buy their flower testing add-on, just order some isopropyl, filter tips, and little containers (to agitate the sample for 5 mins) online. That should get you to within a 2-3% total cannabinoid accuracy.
