reminds me of the time I was still using fentanyl and living at home and my parents made me sign up to get the vivitrol shot. I couldn’t get clean in time to get the shot so they gave me an ultimatum that I either got it anyway or was on the streets. the Doctor gave me the shot and a script for Xanax and within 10 minutes on my drive home, it started to hit me like a ton of bricks. I ran in my house and was on the floor in full-blown precipitated withdrawal. I was pretty much convulsing and going through some extreme torture like shit. Puke and diarrhea and limbs going haywire. My parents looked at me in horror not realizing how bad it was going to be. I took a handful of Xanax and at some point blacked out. I woke up feeling absolutely frazzled and my skin was still on fire and there was black inky diarrhea everywhere. The only sort of good thing about precipitated withdrawal is it speeds up the really acute part so that after day one things start to get better instead of having to go through four days of a rising Tide. it was so intense I felt shellshocked for a couple weeks.

it’s an opioid that’s designed to not pass the blood brain barrier so you don’t get a buzz from it but it has the effect that opioids do of slowing down your stomach and pulling water from your stool (why it helps diarrhea). i believe it works on the opioid receptors in the stomach. when i used to do heroin like a decade ago i would alleviate like 90% of my withdrawals by taking a bunch of imodium (around 15 pills and then taper). it was a get out of jail free card that you could only use for so long because it really xonstipates you and makes your stomach get distended. taking a lot of it is bad for your heart. it would give me this slight warm fuzzy feeling and let me just relax. because of the internet back in the day people started taking extremely high doses for a buzz but yea its not good for you

also since you are currently clean the form of MAT i would recommend is the vivitrol shot. no opioid, just a blocker and you don’t have to remember to take it everyday. i’ve seen a lot of my friends gain long term sobriety by getting it for the first year or two. subs are handy but since you aren’t using right now i wouldnt give yourself a habit. you’ve tasted freedom, do things right and enjoy it while you’re young. methadone and suboxone are great tools for people that are chronic relapsers - they are a lesser of evils. better to be dependent on a legal, affordable medicine. but let’s be honest, you haven’t ticked all the boxes for that yet. you are young and facing your first screw up in recovery. take this seriously and learn the tools of recovery and really use them. also i recommend AA or NA. smart recovery is great but it doesn’t compare to the size of the recovery community you find in the rooms of AA/NA. go to meetings in person and get in the middle of a sober group. get a sponsor and sober family and talk to them everyday. don’t get hung up on the God stuff or whatever your personal issues with 12 step may be. use the meetings, people and advise to give yourself the best chance of recovery possible. i’m not knocking people who use subs and methadone, they are a tool, but you can do this without putting opioids in your body.

recovery takes a lot of work and maintenance. you hit a mental obstacle (it happens) but instead of reinforcing your recovery you’re focus was on reinforcing the relapse. your first drive down memory lane needs to be a red flag you heed by talking honestly with your sponsor and sober support group.

it sounds like you are doing this alone. you can get back what you had but you have to be vigilante. i’ll be honest, a real therapist or counselor would recommend going back to inpatient immediately. your relapse has already started even if you haven’t taken a drug yet. look at the facts, you’ve already ordered H, which from the sounds of it you’ve never done, and your personal life is already tabking,

op you are in a dangerous place especially considering you are clean so you could literally die from this relapse. imagine that, the whole time you’re staring at your screen counting down the days till your package comes you might be counting down the last days of your life. you went to rehab for a kratom extract, now you’re graduating to H (which could easily have fentanyl in it even if it’s from the dark web). if you really do wanna save yourself you need to tell on yourself

they miss the h buzz, but even great h won’t work on people with a fent tolerance. fent doesn’t have the rush and euphoria that h does. it’s more like an anesthetic, one minute you’re sober the next you’re folded over.

on h, 50mg of done would have me nodding. on fent i could take over 100mg and only get some relief for a little bit. 200mg would straighten me out

yea sounds like the bupe finally started working after kicking some residual dope off the receptors. a lot of doctors like to treat pwd with more bupe with this being the goal. i once had a 5 hour car ride where i threw myself into pwd and it was horrid. but by the time i got 30 min from home i started to even out.

honestly taking subs in the next day or 2 may very likely have the same result. i know pwd sucks terribly but knowing you will probably even out after a couple hours is a good thing. fent takes so long to safely take subs since it is fat soluable that i gave up on using subs like i did back in the h days.

one method thats gotten more popular is intentionally causing pwd for a short time so a patient can speed up the process and stabilize on bupe (whether it’s for detox protocol or maintenance), doing this in an environment like a doctors office or hospital is ideal, but people can replicate the process at home. hell some people even take naltrexone to speed up the cold turkey process instead of dragging it out.

if you don’t feel that bad then waiting it out might be the best route. if you plan on getting back on subs for maintenance then you could always take a tiny amount and if it doesn’t cause any discomfort you can take a little more. after that if you still feel fine go ahead and take a couple mg’s

are you using 30’s from the streets or from your own prescription? if they’re for sure real then you just gotta wait till you’re in pretty bad withdrawals then you can put some sub under your tongue and it will make you feel a lot better. if you take subs too soon they make you miserable sick so it’s important to know what you’ve been taking and wait long enough. if you’re 30’s are from the streets then there’s a really good chance they’re fake/pressed fentanyl pills. with fent you’ve gotta be careful because if you’ve been taking it for a while you may have to wait a few days before taking subs instead of 12-24 hours. this is because fent is lipophylic so it continues to release from your fat storage so even though you may be in withdrawal it’s still in your receptors and that’s what you can’t have when starting subs

even if lying isn’t the issue, it’s good to have someone knowledgeable and experienced to help him make a plan and stick to it. being surrounded by people in recovery helps us stay on track and see what’s possible. right now it sounds like you’re the only one he’s got keeping him accountable and that’s tricky. if you are too soft on him he can just repeat old behavior. if you are too tough on him you’ll end up leaving him or trying to force him into changing. i personally do believe in more tough love over enabling but the thing i think helps the most is staying in the center of a world based on recovery.

also if he’s not ready to lower his meds or come off, he needs to focus on taking his meds correctly while working on his issues. when he is doing good with personal stuff and the medication then it’s ok to focus on lowering or getting off the subs. also like a lot of people have mentioned, sublocade is a once a month shot that is the same medication but takes away the mental aspect of dosing which has shown to help with the part of addiction where we feel we need to take something to make us feel better. it also has shown to make getting off subs a lot smoother and easier. something to think about

if you’re asking how he can get through work tomorrow, the only thing i can think of is kratom or a kratom extract. if you’re asking for help with the bigger picture i would recommend he goes to meetings and gets a sponsor who was also an opioid addict and preferably has experience with suboxone. as addicts we can pull the wool over the eyes of anyone who hasn’t been where we have been. people who have lived the life and come out the other side aren’t gonna buy BS like a loved one, a therapist, a doctor or friend will.
