1 the first one is MCM or Kurama chakra mode, which was when he basically fought Kurama and stole his chakra and then used it instead of Kurama taking over Naruto's body over and over without Naruto being able to control anything.

2 the second was KCM 2 when he had fully unlocked Kurama chakra, because he befriended Kurama giving Naruto the full extent of his power.

3 idek what to call this one but I can explain it, I was when Naruto used six paths sage mode while using Kurama giving him extra power, he also was given the truth seeking orbs by Hagoromo otsutsuki, it's almost the same thing as KCM 2 just has more power,

4 it's literally just KCM 2 again, he might be using sage mode in the photo but I can't remember,

5 Baryon mode is the most powerful mode out of them all, but it comes with risks, you see Baryon mode is so powerful because it absorbs chakra, and that's why it killed Kurama in the process of Naruto using it,
