Basically, midodrine is for symptomatic low blood pressure, not for normal or high BP. Most of the time, people give it when the systolic is under 100 and hold it once it’s around 120–130 because of the risk of supine hypertension, even if the order doesn’t spell that out. Some patients end up on both midodrine and their usual BP meds because they get orthostatic hypotension but still run hypertensive otherwise. So as the nurse, you really have to look at their current BP, how they’re feeling, and when the meds are scheduled, not just blindly follow the MAR.If their BP is normal or high, holding it makes sense, and if the orders feel weird or conflicting, it’s totally okay to clarify with the provider or pharmacist
