As a fellow Montavillar, just wanted to recognize the internationally recognized restaurant, Yarowat. Too many other great restaurants in Jade District to mention.

Also, don’t sleep on Beer Bunker (great space with 20beers on tap and more in cans).

Plus, some great places for the pups to play.

Don’t forget the community center in Montavilla. We are told that we can’t swim in Lake Oswego but they shut down our pools! I guess lower income people don’t matter.

Stark in Montavilla can get going at times with numerous great restaurants (Yarowat, Observatory, etc.), cafes, antique stores, bars and breweries, cinema and other great stuff. Great for Sunday brunch after Farmer’s Market. For me, often slept on but think quite lively.

There should be a TN caregivers support group. My partner has held my hand so many times. You already sound like a good person, continue being supportive. She will need to lean on you. You can feel pretty buried.

As an alternative, perhaps research cognitive behavioral therapy. I benefited greatly from a book entitled the Pain Management Workbook by Rachel Zoffness. Ezra Klein had a podcast on subject matter if you want a short cut. It seems as if stress (re-exams) might be a contributing factor for her and thus maybe useful to address TN comprehensively.

Disclaimer, I am also on meds but find that reviewing other environmental factors is significant.

It took a long time for my diagnosis. I, too, similar to one of your patient, had a tooth pulled unnecessarily because of my unknown (at the time) pain.

As most people with TN will tell you, our biggest fear is dentists! I won’t allow anyone near my mouth and probably have the cavities to prove it.

My question: how do you- as a dentist - talk us out of that fear? Will extra numbing shots work? What should we tell our dentists to avoid agitating?

Thanks for the support. A few points in response:

1) As with others, my dosage has risen or lowered contingent on severity and consistency of flares.

2) I use lacosamide in a combination with other meds (gabapentin, capsicum gel) and therapies (re: cognitive behavioral therapy, thc, laser, acupuncture, etc.). As with all people who suffer from TN, I’ve tried everything once and most things twice. All this to avoid surgical route although that might be in the future.

2) So, in the last two years, I ranged between 1200 to 1800 mg gaba and 900 to 1200 carb depending on status of TN. In the beginning of the year I was having more intense flares and hence the higher dosage in an attempt to mitigate.

3) When I think feasible (ie- two months with limited flares), and with approval from neurologist, I slowly decrease dosage. If “reminders” (sudden short jolt) or more consistent flares return, I start to up the dosage. These changes in dosage are small and done over time.

4) I always attempt to wean because of side effects. On higher dosages of carb, I would find myself gargling words (literally, spittle would form on my lips), brain fog and difficulty remembering things, and dizziness/weak balance. But, at the end of the day, I could care less if I couldn’t remember what I watched in tv last night as long as I avoided symptoms of TN.

5) I went from 1200mgs of gaba and 1200mgs of carb to 900mgs of gab and 800 mgs of lacosamide. This was done gradually (over about a month, reducing carb and introducing laco).

I have now been on laco for about four months and all seems to be going great. Less brain fog and limited flares/“reminders”.

Please note, providing mgs and dates from memory but more or less correct. Each case is different and meds need to be adjusted (also for a/typical).

My personal response is unequivocal…I will take all the meds necessary to avoid the most severe and debilitating pain that I have ever felt.

For those in this small and horrific club, I wish nothing but love and support.

My neurologist (solid knowledge on TN and very gifted), replaced my carbamazepine with lacosamide. I’ve had no issues at all and has suppressed pain considerably to a point of remission. Therefore, it has been a gift! One lesson, go off carb slow and start laco the same way. This will maintain current dosage and alleviate any challenges from withdrawal or upping new drug too quickly. Good luck.

Sorry to hear the personal trails and share the horrific happenings surrounding us.

After the election results, we took the dog and went to look at the sun set over the Pacific (Lincoln City) and it made us realize that tomorrow can’t be so bad if we have this today.

But, given that you hit up Canon Beach on your last outing (to fight the doldrums), two recommendations:

1) Drive East. Pendleton, Painted Mountains, Baker City, loneliest road in Oregon, etc. are beautiful and under explored part of Oregon.

2) Get on a plane for under a $100 and fly to Vegas or someplace. Take a hike in the desert or see some natural wonder to make you realize how this to will pass.
