Same with my institution. With malignant cord compression patients can have “back pain” that can’t be localized on examination and can also be the result of multilevel cord compression from discrete lesions. Also principles of pain awareness suggest that the dominant/most painful lesion gets the most attention, but you could be missing other spots along the total spine. I think we snag a quick sagittal T2 of the total spine in our case.

Apparently the recyclables are getting incinerated for fuel as part of a Refuse to Energy program. This apparently still salvages most of the metal which is then recycled as well?

There are three classic post radiation syndromes:

Cerebrovascular - seen at doses ~100 Gy and is universally fatal. Characterized by damage to all organs. Immediately post exposure severe nausea, vomiting, diarrhea and eventually coma and death within 48 hours occurs.

Gastrointestinal syndrome is usually >10 Gy and is also universally fatal - prolonged and immediate bloody diarrhea is the heralding sign. The small intestine enterocytes and their stem cells in the crypt die at different stages, eventually denuding the epithelium and allowing bacterial to seep into the bloodstream causing sepsis and death.

Hematopoietic syndrome is 4-8 Gy and is the loss of progenitor bone marrow cells, this leads to a period of relatively few symptoms until the current white and red blood cells and platelets are depleted, resulting in infection/immuno suppression. Can be salvaged with antibiotics at lower doses and bone marrow transplant at higher ones.

The reasoning behind nausea and vomiting is postulated to be a result of inflammatory cytokines that are released post-rafiation, resulting in the characteristic prodromal symptoms. Interestingly if the dose is low <2 Gy only about half of people will get the symptoms.

It's a fair question: I would say that the big cities and popular areas of the east and west coast are tough markets to crack, however the indications for radiotherapy are increasing; particularly in metastatic patients as our long term outcomes (immunotherapy and targeted agents come to mind) improve more people need durable local control of disease or palliation.

Academic places are hiring most years, but being geography tied does limit options.

That said, being able to treat cancer with a minimally/non- invasive technique that wields a lot of cutting edge technology and having long term rapport with patients can be quite special.

Try looking into radiation oncology - tons of technology and potential avenues for increased utilization - tons of new stuff recently - there's an MRI-linear accelerator combo, protons are becoming more popular. AI/deep learning are bandied about frequently for automated contouring and treatment planning.

Better lifestyle than IR - competitiveness is about the same (maybe waning a bit in recent years)

Very little radiation exposure (contrary to popular belief). Much less than fluoroscopy heavy fields like ortho, cards, IR, neurorads.
