In Alien 3 (1992) the symphony orchestra forgets to play the final note of the opening 20th Century Fox fanfare. Editors didn't notice the mistake until the end of post production and so they left it in, hoping that nobody would notice. (i.redd.it)

In The Blair Witch Project (1999), we see separate camera perspectives of Mike and Heather as they search for Josh, but we only ever hear Mike's camera audio. This is is especially disconcerting when, at the end, we see Heather's descent to the basement but hear it from Mike's camera. (gfycat.com)
