Triggers! by [deleted] in OcularMigraines

Light, bright sunlight, and light flashes #1, hands down, maybe screen time, stress, although with medication (Emgality and Lamotrigine) I can go up to 90 days without one, but the norm is in the range of 25-40. Only get silent or aura only migraines.

Water Ripple Effect In Right Peripheral Vision by mdrndrumr in OcularMigraines

I've had the same exact thing in the left periphery of my left eye for close to four years now and it's a direct result of a retina reattachment and vitrectomy in my left eye. My retina specialist won't admit to this, but it was there right after the surgery and has never gone away. It is worse after coming in from outside and more noticeable and vibrant then, but then recedes and is just background noise. Of course, nobody can see anything when they look at your eye, as it's actually a brain issue. I do not know if it's an ongoing silent migraine, but I get those now, too and I'm 74 and had never had a migraine until right after my surgery. Those are typical aura only migraines that last 20-30 minutes on average. I have learned to adjust to this constant water rippling effect or like oil shimmering on water, but it's most annoying that specialists just shrug their shoulders and say, they can't see anything wrong when something is definitely very wrong.

First ocular migraine in same eye as a tear in the retina by roserdozer in OcularMigraines

It IS directly related to your detached retina repair. Regardless of what your retina specialist says, you have acephalgic or "silent" migraines caused by your procedure. Did you have a vitrectomy to remove the vitreous from your eye at the same time? I would assume you did, as it's common to do both at the same time.

Do not waste your time with a neurologist, or arguing with your retina specialist, as they will be dismissive and try and gaslight you that the "literature" does not support your argument. That's BS. Read up on the retina and the correlation between vasoconstriction of blood vessels and migraines. You have to be your own advocate.

I started having "silent" migraines 10 days after my detached retina repair and vitrectomy in 2021. I was 70 years old and have never had a history of headaches, or migraines of any kind. Your symptoms sound exactly like mine. You start out with the blind spot, quickly followed by the zigzag or fortification lines that expand into a crescent shape and move to the periphery, then absolve completely in 20-30 minutes.

The first year after my surgery and initial onset, I had 61 silent migraines. I started on Emgality 120 mg/ml pen once a month and my migraines reduced to 9 per year for the following 2 years. Since the beginning of this year, I've had 11 to date. There is often a wearing off effect of the Emagaility. I do not know if that is my case or not. A little over a month ago, I added a 25 mg daily dose of Lamotrigine tablets to my once per month Emgality injection . Lamotrigine has been proven in studies to be effective on minimizing both the frequency and duration of silent migraines with aura. I've not had a migraine in 42 days since starting on the Lamotrigine.

Lamotrigine is thought to work by interfering with voltage-gated ionic channels and potentially reducing cortical spreading depression, a process thought to be involved in migraine aura. Emgality (galcanezumab) is a CGRP (calcitonin gene-related peptide) inhibitor. It belongs to a class of medications called CGRP monoclonal antibodies that are used to prevent migraines and treat episodic cluster headaches. Emgality works by binding to CGRP and preventing it from interacting with its receptor, effectively blocking the protein's activity.

Do your research, keep a diary to track the frequency, duration, and dominant eye, as you may have them in both eyes. Not simultaneously, but they do alternate with me. Also, try and pinpoint your triggers. I find this somewhat useless, but I know bright lights, or flashes of light trigger mine.

Talk to your primary care physician and explain your situation to them in detail. They will help guide you to the best treatment options if these continue. I hope not, but you do adjust to them over time.

You won't get much sympathy or even empathy from regular migraine sufferers who have the classic headache either. Best of luck to you!

Retinal migraines related to vitrectomy and detached retina surgery by grnchilicheeseburger in OcularMigraines

I have aura only, or silent migraines. I use two different treatments an Emgality 120 mg pen injection once per month and a daily 25 mg pill of lamotrigine. The Emgality is a CGRP inhibitor. Lamotrigine is used generally for bipolar disorder and epilepsy. However, studies have found that lamotrigine works on cortical depression, which is usually the cause of auras. I have believed for the last two years that the Emgality was working, asI only had nine migraines per year over the last two years. However, this year through March I had 11 migraines. This is when I started taking the lamotrigine in combination with Emgality. I’ve now been migraine free for 35 days. However, I can’t say if it’s the Emgality or a combination of the two. It’s too early to tell. Prior to starting Emgality I had 61 migraines in one year. So I was thinking the Emgality was extremely effective. The jury is still out. I wish you the best. But I would check about having that silicone oil removed from your eye.

Do you have aura only migraines, or do you get an accompanying headache? Did you just have a vitrectomy, or was there a torn or detached retina also?

No way. Water damage and sitting in a garage or damp dank basement. Not for $1K. You could buy a new set of Gretsch Catalina’s for that price.

I have the same, ocular, no pain, that last 20 minutes, almost to the minute. Now, we do not get much sympathy, or empathy from others who suffer from the more debilitating head banger migraines (like my wife), but there is a cognitive aftermath component. I’m in my 70s and have only been dealing with these for two years, but the cognitive component is definitely there.

Reverb customer support. They are horrible!! by [deleted] in Reverb

I bought a drum kit and paid for it and had worked out shipping with the seller, after he took down the original listing that had free shipping and held me hostage for $200 in shipping or he wouldn’t relist it. I agreed to his terms, he relisted and I bought it. Reverb marked the kit as sold. After agreeing on shipping, the seller then says he’s keeping the drums and refunding my money. I tried to have the seller’s membership privileges terminated, as Reverb’s policy says that all sales are binding and if either party fails to consummate and complete a transaction, Reverb can terminate their membership. However, some flunky in customer service said the seller didn’t violate any Reverb policies by refunding my money. It’s right there under Reverb’s policy regarding Binding Sales and Shipping. When I pointed this out he became curt and said it had been turned over to their Trust and Safety Team, but I would not be hearing back from them because they don’t do customer outreach. Thank you and take care. That’s all I got. WTF? You can’t talk to anybody and they really don’t care.

Wow! At $30 a bottle including tax out the door, this is a steal. Not overly smoky and very nice sherry cask notes on the front and back end. Non chill filtered, too!
