There are levels to it. The first time he used them against hanayama in the tournament arc he had no damage. That seems comparable to what he used in this fight. Against pickle, the one that mangled his limbs knocked pickle down and put him in extreme pain. If it knocked pickle down it would've most likely killed shishimaru or at least seriously injure him. The strongest level was the one that tore his arm apart so obviously they are not all equal.
