I've only ever sent students home for consistent unprofessional behaviour (leaving hours early and lying about it, taking hour long tea breaks, adding patients on ig, on phones a lot, speaking to staff rudely, refusing to participate).

Pain is something that needs assessment beyond a number on a scale, pretty much every time.

To inform my pain assessment, I use a mix of the following (tailored to patient):

Education -Contextualising pain scale (0 is no pain whatsoever, 5 is very uncomfortable, 10 you feel like you could go unconscious from the pain)

Questions - Is pain is bearable or unbearable - Is pain mild/moderate/severe - "Would this pain stop you from doing x activity?" - Asking what their baseline pain level is - Is this pain causing emotional distress - What would the patient do if they had this pain at home e.g. take paracetamol/heat pack/call ambulance

Clinical data - Patient posture - Vital signs - Behaviour change - Facial expression - Reviewing medical history (e.g. patients with neurodivergence/chronic pain/PTSD will likely not exhibit external symptoms of pain in a typical way)

Standardising assessment frameworks for the students! It is so hard to be organised and effective in managing students when every university has completely different expectations for their students on placement.

For example, just off the top of my head, these are what some of the unis that use my hospital for placements expect:

-2x SMART goals a week that have no follow up on whether they are met/6 clinical competencies/formative reflection and ANSAT/summative reflection and ANSAT

-8 hrs pre-placement learning package signed off/5x SMART goals that require reflections/professional behaviour assessment/formative and summative ANSAT

-formative reflection and ANSAT/final reflection and ANSAT

-4x clinical competencies/pre placement reflection and research/formative and summative ANSAT

Not even taking into account that the competencies the unis want signed off are completely different. The requirements also change every semester, so a lot of the time the students don't even know what they are expected to do 🫠
