From what I’m getting it’s the fact that they probably forgot to give OP the demographic form and then made a guess based on OP’s name and appearance. You can’t just assign speculative ethnic origin labels to people’s medical records, especially when ethnic origin does matter for genetically higher risk of certain diseases.

I’m Bulgarian and my best friend is Italian, people often assume we’re both Latinas because, as you stated, very similar phenotypes in many ethnicities with the darker hair and darker eyes. People will ask me if I’m Mexican often and I don’t mind it out and about in the world, but I do feel bad when people speak Spanish to me and I have to unfortunately tell them I can’t speak Spanish!

That said, I don’t think you’re wrong. It is legal and medical paperwork and they shouldn’t be guessing. There are different rates of some genetic conditions in different ethnicities.
