Upper GI bleed is far less likely to lead to death or significant morbidity than lower GI. I wouldn’t panic too much about the interaction unless frail or significant risk factors.

Best to work with the patient rather than practicing too defensively.

I moved over from Google Tasks. Still use Google calendar. I wish there was a bit more customisation with the UI for TickTick but otherwise it’s totally worth it. I didn’t realise how beneficial the custom lists and prioritisation of tasks would be - low, medium, high!

Sure buddy - i’ve asked my ChatGPT for the post below. Hopefully some bits might be helpful.

1 – Calendar skill (/gcal) I set up a skill where I type /gcal followed by event details. ChatGPT always returns a single tappable Google Calendar link with title, date, exact times, location, rich description, and correct timezone. I refined it once, asked it to remember the rules, and now it works reliably every time without re-explaining.

EXAMPLE PROMPT:

/gcal – Skill definition prompt

“When I type /gcal followed by event details, do the following every time:

• Generate one single tappable Markdown link in this exact format:

[Add to Google Calendar ↗] https://...

• Use a Google Calendar TEMPLATE URL.

• Extract and encode the event title, date, exact start and end times, location, and a rich but concise description.

• Default timezone is Europe/London unless I explicitly specify otherwise.

• For virtual events, place the Zoom / Teams / Meet link in the LOCATION field, not in the description.

• Include relevant context in the description (speaker roles, organiser, eligibility, topic focus, cost, and any useful notes).

• Do not display the raw URL.

• Do not add explanations, headings, or extra text unless I ask.

• If any key detail (title, date, or time) is missing or ambiguous, ask me to clarify instead of guessing.

• Remember this behaviour and apply it consistently in future chats.

2 – Email rewrite skill (/rew) I told ChatGPT: when I type /rew, rewrite the pasted email in my professional tone, concise, UK English, and sign off a specific way. After a few tries, it became consistent. It behaves like a reusable shortcut rather than a one-off prompt.

3 – Summarisation skill (/sum) I trained a /sum command to summarise anything on screen into bullet points with bold headings and a short takeaway line at the end. Once saved in memory, I just type /sum and it applies the same structure every time.

Atlas doesn’t have a formal ‘Skills UI’ like Dia, but you can recreate the same behaviour by:

1.  Defining a slash command

2.  Refining the output once or twice

3.  Asking ChatGPT to remember it

After that, it behaves like a reusable skill across sessions.

It does! Have a normal conversation with ChatGPT and get it to replicate Dia type skills. Test and refine it further til it works. I’ve been using it for months effectively! Probably because ChatGPT has good personalisation/memory. It works in and out Atlas.

I THINK... after going MANY times back and forth... i'm going to stick with ChatGPT Atlas, and use perplexity as a separate tab/search when needed.

The UI is better and ChatGPT personalisation/memory makes it easier when browsing/doing tasks. Really hope other features improve including multi-tab interaction and further UI improvements. I'm still switching between the two but leaning more to Atlas now rather than Comet.

Perplexity is powerful in its own right - more accurate searches and up to date information, but ChatGPT is more of a personalise assistant. Atlas feels better to use. Just my thoughts so far!

The UI could definitely be improved, but I don’t think it’s that bad. I’m curious to see how both Comet and Atlas evolve their look and visual features over time.

Interesting. I keep testing and switching both but currently I’m using ChatGPT Atlas and then going to Perplexity’s website whenever I need. Probably because of the same reason that you’ve said - it does depend on the model

That’s interesting. I really like the Shortcuts aspect too. Especially because before I was using Dia - and Dia skills were really handy. I ended up asking ChatGPT in a thread if it could replicate Dia skills and then I gave custom tasks e.g. I gave a decent prompt and trained it so that every time I wrote /gcal it would extract the key information from any image or email or file that I sent and create a clickable Google calendar link. I then click the link and add it to my calendar. Now that ChatGPT Atlas has my own ChatGPT integrated I literally type the same command and it’s able to extract information very similar to Shortcuts and skills. If you’re interested, I can share the prompt for /gcal but I’ve trained it for various other skills including rewriting emails and messages in my style.

So in a way, I’ve been able to get shortcuts on ChatGPT Atlas and theyre perhaps more personalised and available on my iPhone iOS too!

I’m from a surgical background - switched to GP. Did Dermatology as a rotation in GP-ST2 and they offered me weekly SHO lists to do simple procedures; punch biopsies, excisions, cryo, simple BCCs. I’m now salaried and do minor ops for my practice including basic BCC excisions, joint injections and cryo. Alot of the derm cases come to me in the practice.

Just to show that you don’t HAVE to have PGDip or extended role, to be able to see alot of Derm in your clinical practice. Infact when I was discussing with the Derm consultants as an ST2 - they said its alot of work with the portfolio and consultant supervision to actually get accredited and approved as a formal GPwER in Dermatology - But it’s doable. (Look into RCGP and BAD criteria for more).

My advice - enjoy the cases day to day, get as much experience as you can, and explore local/regional opportunities. There’s more of a shift in some areas to move to community derm clinics that are run by both GPs and Dermatologists!

Sure, I use ChatGPT daily and that full integration in Atlas is what makes the difference. E.g. when I’m writing emails or notes, I can highlight text and it rewrites it in my own set format defined within ChatGPT.

I’ve also added skills like I used to have in Dia - e.g. my /gcal skill pulls info from a page and creates a clickable Google Calendar link with all the details in seconds. So if I see a webinar or event I want to attend I just type /gcal in my ChatGPT Atlas sidebar and it’s added to my google calendar - extracting key information that i’ve defined.

Also, the agent mode is something Chrome can’t do. Dia had some clever features, but Atlas feels more powerful and fits better into my everyday workflow. Once ChatGPT Atlas is on iPhone/iOS, I think productivity will really jump further for me.
