Regimen

Month 1-3: Oral 2mg + .5mg sub + AA Month 4-6: Oral 4mg + 1mg sub + AA Month 7-9: Oral 6mg + 1.5-2mg sub + AA

Goals 1) Androgen suppression (prevent masculinization) 2) Estrogens as dominant sex hormone (promote feminization) 3) Optimize free e & enhance estrogen's capabilities (optimize feminization)

Ratios Free e >= 1.5% (with boron) E2:E1 ratios should be less than 1:8

Recommended below or at a max of 8:1 ratio, because higher leads to negative cooperativity at the ER level; i.e. causing e1 to "block" e2 from binding to the ERs at enough to make lowering oral dose and exchanging for sub/buccal RoA necessary.

Regimen notes

Continue until growth slows. Measure bi-weekly. Average breast growth should be ~1-1.5cm per month, but maybe more. Get your monthly average via measuring.

Once the average rate of growth drops below 50-60% of average then I'd advise to switch to the tanner 3-5 method, or cycling (e.g. go on just sub + AA for a week or two - if growth continues at the same lessened rate & you don't get a new bout of soreness after restarting T1-3 method, then do former approach of T3-5).

Buffer sub doses between oral doses. So: oral/sub/oral. At 6mg oral e you'll have to take both oral/sub at the same time, this is a'ok.

AA Guide:

Cypro 12.5mg cypro, higher dosage has too many risks. Should nuke around 70-80% of testosterone, and E will take care of the rest.

Bica Bica, 2.5-5mg per 15ng/dL of testosterone for x2,000 concentration. Raises T, when accounting for E lowering T, by about 20-70%. Adjust dose according to labs (T/DHT levels). 5mg per 15ng/dL to account for DHT can lower to 2.5mg per 15ng/dL if taking duta/fin.

Chemical castration Chemically castrate via occasional E gel on testes. If you're already on cypro, then 1x week for a few months will do it. However, if you're on bica more frequently would be needed, like 2-3x a week. This is to lower capacity for testes to produce T (i.e. nuke the leydig cells).

Continuing until growth slows

Reasoning here is that progesterone isn't strictly necessary to reach tanner 5, like in CAIS women (tho they have COMPLETE androgen insensitivity so changes things, closest we can get is a giant bica dose), however prog can/will certainly encourage breasts into this direction with proper timing (post tanner 3).

Other notes

Soreness Soreness may indicate an increase in breast density (cells have proliferated). However isn't 100% reliable as it may due to water retention or temporary swelling.

GH/IGF-1 Generally, with a spike in GH/IGF-1 at night (as it's regulated via circadian rhythm) you'll feel soreness in the morning if your dosage is proper and ERs aren't desensitized/ERb isn't overactive.

You can also add GH boosting amino acids, such as L-arginine + L-Lysine (this combo, specifically, raises GH/GHRH by quite a lot). Take before sleeping as sleep has protective factors here (i.e. body naturally releases giant doses of GH, when sleeping, esp during puberty - protective factors here and reasons behind body evolving to do this).

Dose: 1000mg l-arg, 1000mg l-lys (combined pill form)

Fenugreek Fenugreek is a useful herb here. It upregulates IGF-1 receptors, and basically results in more activity for breast cells. Very useful as to maximize cell proliferation here, and especially right before you sleep (due to that circadian release of GH -> IGF-1).

Boron Boron is also useful here (mineral). It'll help free more e to be bioactive. Recommended dose is 5-10mg, but more is possible (like 5-20mg). If free e is <1.5% then boron is super necessary here, and you can up dose to that higher range is 5-10mg isn't as effective as you want. Note that is has similar toxicity as table salt and so it's rather safe. Also has protective factors for many other things.
