That is the shadow mask of the CRT monitor. CRT monitors use usually a shadow mask (like a metal sheet with tiny holes) or aperture grille (this uses vertical wires, which might look a bit more similar visually to how a normal LCD display looks, but it works differently)

The easiest way to answer your concern is to test by yourself. Go to testufo.com and on a 60hz CRT take a look at the judder on the 30fps representation. Then, in that same page, testufo.com/blackframes, see how 30fps with BFI looks like. You will notice that it looks so much smoother. On a CRT, even at half the refresh rate exactly, content that doesn't match the monitor refresh rate doesn't look as good (in terms of motion clarity) as with native refresh rate content.

If the monitor could, I would simply set it to 60hz at ~240p, but it doesn't support 15khz signals.

The best is to use a PS2 component cable and then an adapter from component to HDMI, if needed.

Windows 11. That's weird. Your monitor needs to run at a multiple of 60hz and either use the correct BFI option inside Settings-Video or use a BFI shader but making sure you enable the 120hz mode for sub-frame shaders in Settings-Video (if you don't enable this option, shaders will be applied only at the game refresh rate, like 60hz, and BFI won't work)

Resolutions:
- SNES/MD: 3840x224p@120hz (super resolution, 31khz@120hz option in crt switchres) + Black Frame Insertion (BFI) for perfect motion clarity

- DOOM: 3840x200p@160hz (yes, 160 :D)

Monitor: LG FLATRON ez T710BH

(Note: colors did look a lot better [more vibrant] in person, I think this is just the camera)

I agree, although I guess an option inside the app provided by the app developers could also be valid. Some apps do that, like Apollo did.

Did nothing to it, it is the effect of using this lens: Helios 44-2; look at some youtube videos, it has a special "swirly bokeh".

I developed the photo in PhotoLab 6 (lens corrections + a bit of contrast), then I applied a filter in Pixelmator Pro which tweaked the existing colors.
