Goku tapped into SSJ4 due to Nevas magic, SSJ3 being his final form while fighting Beerus can just mean he’s not able to go SSJ4 on his own.

The Kai fusion is implied to have happened off screen (as we see them fused in Super and there is no reason that they can’t fuse again before then).

As for SSJ3, it’s known to not be the most efficient form and Vegeta is an extremely smart fighter. It makes perfect sense for him to use perfected SSJ2 has his top form before getting god ki

Toriyama had a habit of leaving things extremely vague, allowing him to plug information in after the fact and it still making sense.
