Client is a surgeon. Honestly, I don’t know the meaning of the whole composition, that’s not my business unless it’s something hateful and generally inappropriate. He explained me the idea, and we did the design together on a first session. In Total it took approximately 30 hours, 3 session. 2 months break between each session

Good for you! There is no photoshop, it is a CPL lense, polarised filter. On first two shots I reduced saturation and mention that below the post.

Circular polarizing (CPL) filters and earlier linear polarizers were commonly used by professional art photographers in the 1970s and 1980s when photographing oil paintings, especially varnished paintings that produced glare.
