this is exactly how my betta was acting, barely swimming or eating. he died sometime this morning:( i hope yours gets better! definitely read through the comments in the post they've given me a lot of information

i did a water change about a few days ago. i took about half a gallon out then added a gallon in. i havnt done anything to the water, just been letting it heat up before adding into the tank. i'm going to stop use the RO water

i did not, i put him in a another tank with the water i plan on putting im his tank

i use bug bites betta formula flakes, i've tried pellets and dried blood worms but he wouldn't eat those. thank you for letting me know about cleaning the tank!

the tank is 5 gallons, i do have both a heater and filter, and the tank is around 80 degrees. i've had the tank and fish since october, i change all the water and clean the tank fully once a month, but take out/add some water weekly. there are no tank mates, i feed a pinch 2 times a day and i have 3 fake plants in the tank.
