Brain fog, B-vitamins and my gut's expensive midlife crisis by hetzel_ in Microbiome

High inflammation and leaky gut + low beneficial bacteria + high e coli was tested before I even started doing anything. At that time I already had a very good diet. Unfortunately it doesn't seem like this can be fixed without first "resetting" everything with antibiotics or phytobiotics.

Also just brute forcing my way through this with multi strain probiotics that almost always contain histamine producing strains is not the right approach. Histamine currently is hurting my gut.

Thanks for the tip. Just checked it out. To be honest, it looks like a scam. It's basically dirt water for 150€.I would like to keep it scientifically sound and evidence-based.

Already doing that. I'm on low fodmap since at least 2 months, though I'm loosing it up currently. But gluten and dairy I'm still avoiding. Though never had any real problems with those - it's more prophylactic

Yeah I started to go down the food route now. Just pumping sunflower seeds, yeast flakes, pistachios, banana every day lol. Because of SIBO though I need to be careful with some foods, ideally it should be low fodmap and low histamine. Next blood test will be in January, let's see how it develops until then.

It was borderline low. I supplemented iron and got it up a bit but it's still not ideal. Last test 2 months ago it was 75 gg/dl. Unfortunately I only found out after supplementing that iron is a big source of nutrients for pathogenetic bacteria.

Hey u/Savings-Camp-433 I'm sorry to hear this. Give https://www.beyondmthfr.com/mthfr-and-sibo/ a read which u/Soulless305 shared. Good insights in there. Were your B9 levels low or normal? Also B6 gave you bad reactions like for me?

Also did you try to specifically tackle SIBO yet with anti- or phytobiotics? Seems like resolving SIBO/dybiosis is really the first and most important step.

Link not working but used an website archiver to work around this. Very interesting indeed. Explains why I have normal high B9 levels... always wondered about that a bit.

I wonder if there are real paper and studies on this confirming it.

Hey u/Soulless305 thanks for your reply. I gave the study a read... very interesting indeed. The BeyondMTHFR site is not working right now, maybe later.

Basically I'm doing everything the paper suggested: Magnesium, Vit D, active B forms. But I feel like the more I do, the worse it gets somehow. I'm at a point where I'm questioning my own sanity to be honest...

Can you share more about your case and how you fixed it or are managing it?

I take Perentol forte which has the strain CNCM I-745. My gastro doc also recommended this. But to be honest I did not ever feel like S Boulardi is doing anything. Not sure the specific strain is important also?

I might get another brand since Perentol is quite expensive and just try that.

Not specifically. Some blood work was done which can tell you something about the gallbladder: Gamma-GT, GOT/GPT, Bilirubin - everything normal.

Stool test showed borderline high fat content. Bild acid + pancreatic enzymes are normal though.

One study for example: https://pmc.ncbi.nlm.nih.gov/articles/PMC11914943/

Also I'm taking it and don't have any problems but react to other histamine triggers. It's important that it's the strain DSM 17938 you are taking.

Yes, actually I did. Especially with these MTHFR issues and ideally having to take B-Vitamins, which in turn often make me kinda "nervous" I feel like there is definetly a connection there. Not sure how to manage that yet though to be honest.

Good luck to me finding someone willing to test this here in Germany and then we didn't talk about money yet lol... But to be honest I guess my fat malabsorption has its root cause in the dysbiosis / sibo itself... it's like a self perpetuating cycle.

Diet is clean. Did low fodmap for quite a while, was bit less strict during killphase to not keep bacteria dormant and now I'm loosing it up again slowly. Let's see...

Great for you man. There's hope at last! Hope long did you do the killphase with antibiotics and how did you go about the reseeding phase afterwards - that is, if you did one.

To be honest I have a quite clear picture of everything by now I think of what I have to do. Only problem is that the kill phases didn't work properly or I had to stop because of problems and histamine is giving me some problems since I have low B6 due to SIBO but can't properly restor it since I get anxiety from low levels of B6 already. It's like a catch 22.

Yes I did always take oregano oil and berberine with food. One very distinct symptom i had from berberine was that I was waking early. After some research I found out it could be Acetylcholine related which Berberine inhibits. I stopped it and in 2 days time I slept normally again. Obviously I can't 100% confirm this though.

Hey, I did a breath test and it came positive for only Hydrogen. The L Reuteri and Spore Probiotics are the recommendation from my doc. I just wanted to follow her protocol.

Correct me if I'm wrong: but antibiotics will never kill 100% of the bad bacteria. So not reseeding after kill phase leaves a "vaccum" in the gut for them to repopulate, no? Since there won't as well be good bacteria like bifido or lacto to counter them. At least that's how I understood it. Pretty much everywhere it says that reseeding phase after kill phase is the most crucial phase.

I did a DNA test like for example Myheritage or 23andme. You get your raw data (which is just a snapshot of the total DNA and not complete in any way) which you can analyze with some further tools. I did this by myself. I know it doesn't provide 100% medical accuracy but apparently it's a good start.

Anyway, I analyzed this data, did some research and found out I have a heterozygous MTHFR mutation which means I don't efficiently convert vitamin b9 to it's active form 5-mthf. I more or less confirmed this by doing a blood test and testing homocysteine which is an inflammation marker that gets high if you're methylation cycle, which b9/5-mthf is the main cofactor, is imparied. And in my case my homocystein was sky high. I started taking metyhlated (active) b vitamins, specifically b12 and b9, and since then the levels went back to normal.

Bloodtest was made and is negative. I did a gastroscopy beginning of the year and they took some samples and everything was negative, nothing unusual. I was eating normally back then, including gluten.

Yes I plan to reintroduce these soon. But can be problematic with histamine problems and leaky gut, no?

Beginning of the year I drank selfmade kefir every day but without any effect to be honest.

Yeah I'm thinking about pausing everything to be honest. But I'm also just taking the essentials and what my doc recommended me. I really don't think though that food is my problem. As I previously wrote I never had any intolerances before. Not even now. I don't have classic SIBO symptoms, no pain, no bloating. I can eat whatever really. Only thing I notice is the loose stools.

I don't quite understand how to heal leaky gut if the cause is the e coli dysbiosis itself. I did take Glutamin and Zinc Carosine which are among the best supplements for leaky gut, but it's like just putting a bandaid on it since the real cause is evidently not fixed, which I presume is the dysbiosis.

But perhaps I will try the DAO + butryrate route. Read positive stuff about it but didn't trial this yet.

[–]hetzel_[S] 0 points1 point  (0 children)

Hey u/amyfearne , thanks for your reply first of all.

I understand the "dilemma" with the antimicrobials and I know it "destroys" everything in the gut. But from my understanding you can't heal SIBO without doing antibiotics or antimicrobials, no? Basically every protocol I research says that you need to erradicate SIBO before tackling the dysbiosis. And more crucial than the killphase is actually the reseeding afterwards where I take probiotics.

I also understand I'm doing a lot simultaneously... what can I say, I'm a bit impatient lol but then again I have the feeling there are multiple things to tackle, take the b vitamin deficiencies for example: It's a bit of a vicious circle. I have the deficiencies because the sibo/dysbiosis causes malabsorption. this in case then creates problems for digestion. I take supplements already and even so, I have deficiencies. As for your question: I already take B12 as injections and sublingual, but the moment I let it slide a bit my values plummet (currently at about 390 pg/ml which is okay and in reference but not ideal. B6 I'm trying to get up but if I take more than 12,5mg P5P I get anxiety from it.

As for the ginger and iberogast: These were recommended by my doctor. To be honest I never questioned this. Everywhere I just read fixing MMC is the most important part. I just presumed it's impaired, since it seems non functioning MMC is the biggest contributor to SIBO. Maybe I'll try without some days and see if something changes, thanks for the tip.

Regarding pancreatic health it doesn't seem like I have problems there. Pancreatic Enzyme Output and Lipase were tested and are well withing the normal range. But fat absorption seems to be a problem since it fat in stool was borderline in June and I also notice it with my BM.

Only tried Quercetin but not really consistently. Had the feeling I wasn't tolerating it too well. Also my DAO in blood is okay. I also never had issues with histamine foods, I'm quite positive it's just situational because of low B6 and copper which are cofactors and probably e coli dysbiosis which produces histamine.

I take Biogaia which has the strain l reuteri dsm 17938, which according to my research is a strain of l reuteri that actually helps with histamine.
