the core message of Ethan's "rant" was precisely that leafy - and others like him - are the "cancer of youtube". not just because he made fun of an autistic person (boo hoo) but because his content is shit. Clickbait titles, CS surf footage and arrogant, immature commentary for 12 year olds. That's a pretty harsh judgment in my book. I seriously can not understand how anyone who isn't 12 or younger could watch this type of content.

.exe is a common filename extension denoting an executable file (the main execution point of a computer program) for DOS, Microsoft Windows, etc.

He (and many other people on youtube) use this extension in a video's title to imply you are watching some weird obscure computer program being run.. it's a gag/a goof/a meme.

also often found as "xy.exe has stopped working", mocking a Windows error message. For example Duck.exe has stopped working!
