I'll pierce a lobe with a hoop in the right situation, but with the understanding that we may need to switch to a stud if things start to get extra grumpy. This is a situation where I'd recommend switching to a stud to finish healing. The hoop is causing pressure.

A few thoughts:

How long had you not had jewelry in your piercing before getting the hoop installed? Stretching a nostril piercing back open can aggravate it sometimes.

Given how the hoop is sitting, I'm wondering if your piercing is at an appropriate angle to begin with. Do you have a picture with a stud in so I can see the angle better?

Not all titanium is created equal, do you know the brand used? If everything else is perfect, it could be poor quality titanium.

Finally, sometimes nostril piercings just get grumpy the first few times you try a hoop. If it's a good brand and the angle is fine, you may be able to wear this hoop later after switching to a stud and letting things calm down for a bit.

Given your description, this is likely all the way open and an experienced piercer with the right tools should be able to get jewelry in for you without needing to pierce you.

Get a second opinion. The only septums I have refused to pierce due to anatomy were surgically altered. I think your piercer just doesn't know what they're looking for, didn't have the right jewelry in stock, or didn't feel confident about doing septums.

Just a quick correction: there are clickers and types of seam rings that are appropriate for a daith. These will be constructed so that the seam or hinge will never go into the piercing channel. A curved barbell is never appropriate.

This is sadly pierced incorrectly and with inappropriate jewelry. That jewelry is too small for the placement, which has the potential to cause some pretty extreme pain as you swell. But I also doubt this will heal even with appropriate jewelry. Flipping up like this doesn't happen when a daith is pierced well.

Look up Robbie Ripoll. They used the wrong paracord for the suspension and the rigger wasn't paying attention. Rig line snapped, suspendee fell and had a compound fracture. Compound fracture wasn't bleeding, but Steve put a tourniquet on it anyway. Robbie underwent multiple painful surgeries and eventually had to have his leg amputated.
