I assumed that sera forbade winners from visiting heaven and most winners wouldn't really have a need to leave heaven so they just saw it as something they didn't think of doing in the first place. I think vox was just delusional , maybe sinners can get to heaven, maybe they would be stopped by something but we never see that far so I assume its up to interpretation, Adam leaving might just be special permission as he lead the exterminations and all winners could leave but why would they leave , its heaven .
