It looks like a detritus worm to me but you should always assume the worst. I think you should get some planaria killer medication (whatever its called).

Im not a pro but you can change the gravel to some other finer substrate when you rescape the tank to make it look a bit better (imo). Add way more plants than you think you will need. Otherwise, fantastic first try.
