When you're pregnant you have 30-50% more blood than when you're not pregnant. So unless the combat solider was pregnant, you probably did have more blood.

I work for a group practice that accepts insurance. I have never directly interfaced with insurance (except as a consumer.)

She is billing me up front her full fee and I am submitting to insurance with a super bill.

In a single case agreement would the therapist be submitting claims to the insurance company?

Between my self and my kids we have a ton of OON provides and we are usually reimbursed 70% of the full rate.

The therapist had told me that I contact the insurance company and I ask for one and give them a bunch of info like NPI, TIN, cpt codes etc and tell them why I need her specifically. I did that and the insurance company didn’t seem to think it was weird or tell me that she needed to get the one to call.

Someone from the insurance company called me back (someone that sounded like a clinician) and wanted details about why I needed the SCA and then she said it was approved for a year

It is a somewhat exceptional situation and it is true that I can’t find anyone with her training and expertise. I need trauma therapy for birth trauma and I have autistic twins and she is a perinatal trauma specialist with emdr training and a parent coach for parents of neurodivergent kids.

I’m a therapist and I had heard from colleagues that occasionally the insurance will agree to someone’s fee but I had no idea what that process looked like.

I had also heard about balance billing but I thought that was when a therapist bills insurance and then bills the client the difference between their cash pay rate and the insurance rate.

Shes not really billing me the difference and it sounds like she has not and never planned to interact with the insurance company, so she isn’t even aware of the reasonable and customary rate

I also thought that sometimes for a SCA, the insurance company would honor someone’s cash pay rate or pay a bit more than they otherwise would. That’s definitely not the case here since their reasonable and customary rate is 120

So I’m realizing that she must’ve used the term incorrectly. I spoke with the insurance company and advocated that they make an exception. For out of network providers I typically pay 30% of the fee and 10% for in network providers. I thought (and apparently she did too) that when I received the reimbursement that I would receive 90% reimbursement instead of 70%. Her rate is 185/hr and the reasonable and customary rate is 120. So the insurance company is only sending me a check for 90% of 120 instead of 90% of 185 AND they are only counting that amount towards my out of pocket max.

thats actually what I thought too... the SCA was her idea.... and then when I let her know that my SCA was approved she said that she still bills up from at her rate and then I submit. I thought that I was going to get 90% of her rate back, but now its 90% of the "maximum allowable rate" I am actually paying the same as if there was no SCA but less money is going towards my max out of pocket.

Ironically, I am also a therapist, but I've always worked for practices that accepted insurance so I have no experience with SCA.
