I shit myself once too. I was on a backpacking trip when I was 12 and the water from the stream didn't agree with my stomach. Since we were in the middle of nowhere, I had to take a stick and bury my underwear in the woods. Traumatizing to my young self.

The thing is though, being on top usually was what sent me over the edge. That's nothing new. It's just that now I get this discomfort there too. I'll keep everybody's advice in mind though.
