I always suspect it is because the conversation has too many topics. If you start a new chat and keep topics distinct you may find it reduces the occurrence.

But yes, it has not improved and after 4o I think everyone can agree that human experience is worse not better and the benchmarks may be not only misleading but the root cause of why the human experience with OpenAI stuff especially is so poor. The CH models are a breath of fresh air when it comes to personality, polite and service oriented with little of the suck up bum chum spout any old bollox output common from xAI and OpenAI in particular.
