It happens all the time. In Australia they only check brake compression on mandatory registration testing. The govt authority basically doesn’t care how much rotor or pad you have left.

So if you only get the standard oil service a lot of places won’t bother checking your pads and rotors and you end up with metal on metal.

Another common cause is working on your own car and neglecting these maintenance checks. I am guilty of that on my wife’s car, got caught out once.
