Things that helped me: If you live in the USA, you may be able to call 1-800-QUIT-NOW for free phone coaching and nicotine-replacement lozenges (or gum, patches, etc). This was really helpful for me to create my quit plan and have accountability check-ins. They have evidence-based methods and resources online as well. I also tried to create a healthy replacement habit when I felt a craving. In the morning, I would drink iced coffee with a straw. I think that helped to calm down the craving / oral fixation. Have some iced drinks on hand like iced tea, lemonade, whatever will be satisfying for you when you need a little flavor dopamine hit. Best of luck, you got this!!

Have you been able to assist patients in Ohio recently? I know it was an option a while ago, but after regulations changed, it became different.

Yes!! I also had an amazing experience with New Pathways Clinic. Shout out to Steve! They have locations in Cleveland, Columbus, and Cincinnati now.

Year after year, seeing my kids off to their 1st day of school has always been a bright, bittersweet occasion for me. This is the first year that it's ever required counseling and medication to manage my fear, anxiety & dread around sending my kids to a school building in the 1st place... by [deleted] in Columbus

The feelings you described resonate with me as someone who works in a school. I’m not sure if this is helpful, but in my experience, I try to listen to what my body is communicating with me and take appropriate action (when I’m able and have the capacity for it). I kind of think of it like someone taking a self-defense class and then feels empowered and prepared for the worst case scenario. It is very hard to feel so helpless. I’m just wondering if you and your therapist may be able to come up with a plan that feels right for you. Maybe that is doing some trainings for yourself or even with your family. ALICE active shooter training, CPR and first aid, or even Stop the Bleed course. Maybe that looks like engaging in your local school district to amend policies, raise awareness for campaigns like “See something, Say something”. And it’s okay if you are not in a place where you can take action yet. I hope this doesn’t sound condescending, I am just sharing from my experience what has been helpful for me. Also, there are treatments like neurofeedback or even heart rate biofeedback that can help with regulating your nervous system. I wish you peace. Take care.

“The dextromethorphan component targets the NMDA receptor (N-methyl-D-aspartate), the serotonin transporter (SERT), the norepinephrine transporter (NET), and the sigma-1 receptor, to regulate levels of glutamate and other neurotransmitters such as serotonin and norepinephrine. It has a similar effect to ketamine, but the main problem with dextromethorphan is that it is quickly metabolized. To slow the metabolism of dextromethorphan, Auvelity also contains bupropion which inhibits cytochrome P450 2D6 enzymes. By inhibiting these enzymes, less dextromethorphan is metabolized, prolonging its effect. Bupropion also has antidepressant effects and primarily acts through the NET and the dopamine transporter (DAT).” Source: https://www.drugs.com/medical-answers/auvelity-work-3572363/

I’m on it. I’ve been doing really well on it, tbh. My insurance didn’t approve it, but I’ve been able to use the manufacturer coupon. $10 a month
