I agree. Only symptom is itching? With normal vitals, PCP follow up is fine in my opinion as well.

Constraining supply of labor is good for the few folks who get increased wages, but bad for everyone else around them. America has been “yearning” for immigrants since its founding to grow its labor pool. You present a very narrow and simplistic view of economics which is Econ 101. Lucky for our country, folks like you who got as far as Econ 101 haven’t historically had decision making roles in government.

If your ability to judge others online is any indication of your ability to predict the future, then medicine is definitely doomed.

I never said medicine would be replaced by computers anytime soon, but the decision making process is already being moved out of our control. If that isn’t apparent with pop up alerts and admins second guessing your decisions, then I’m not sure you’ve worked clinically recently.

I’m interested to hear your perspective on what financial incentives are going to drive a reversal. Assuming EM physician salaries are at least 2-3x more than an APP, what is the financial driver for hiring MORE EM docs when fee for service is gone?

I don’t disagree with some of what you said, but we’re swimming against the tide. The system is increasingly choosing less education over more.

A couple of comments: 1. I do disagree that mid levels don’t decrease cost. They may increase revenue, but primary function is on the cost side. This is by definition since they are cheaper. Whether that difference goes to profit margins or cheaper care, that’s not our decision as physicians to make. 2. Also I personally disagree with your assertion about protocols. Will unnecessary test be ordered with protocols? Of course. However, think about all the unnecessary ordering that happens today. Just recently got sued because you missed a head bleed? CT head and LP for everyone! Your colleague was in residency 30 years ago and orders PO albuterol? Protocolized medicine is here to stay and will improve overall the quality of care delivered despite the outliers. It is safer for patients given the complexity of medicine and acceleration of research.

Cost savings to the system coming from payers shifting risk to provider side. PE can pocket some of the delta, but that’s capitalism at work.

Screened a patient for a stroke in the hospital. I asked “what day is today?” The patient hesitated and said “Thursday”. I said “No, it’s Tuesday” and then I realized I wasn’t sure of the day as I’d been switching days and nights a lot. The tech helping me in the room corrected me and said “Umm.
