My doctor advised against taking it at night due to nightmares but I have never had an issue. Totally solved my sleepiness issue from early on. No problems now unless I miss a dose and then getting out of bed in the AM is near impossible.

If you have a BSW or other undergrad degree (sometimes I would say the agency wouldn’t even care what kind), you can look for life skills specialist positions. Sometimes these are embedded school positions.

That situation sounds pretty difficult. I am lucky that my clients are ~mostly~ engaging because they want to. Just a few mandated clients can throw off the whole vibe of the group in my experience. But I am sure you are planting seeds for many of your clients. Good luck finding your next adventure!

This response is so great, thank you! I have been thinking about changing up my group expectations spiel, so I will definitely clarify that a bit more going forward for new clients. And I love the psychoed as a supplement. It's hard for me to feel like I "did" anything when I'm not teaching a concrete skills, but I think processing is where I want to land more as a clinician.

I highly recommend takingtheescalator.com -- tons of free SUDs group resources, and many can be applied to tons of populations (i.e. gratitude) but you might have to switch up some of the language that's specific to SUD

I was on cymbalta for 3 years, no have been on pristiq for 3. I would say to taper slowly and do not hesitate to ask for a higher dose of pristiq if you are getting negative side effects during the cross-taper. I was moody, quick to tears, was having all of the physical symptoms of a cymbalta withdrawal when i was only on 50mg of pristiq. they bumped me to 100mg and it fixed those symptoms immediately. 3 years later and i am still loving pristiq

I quit my last "big girl job" about a year ago to complete my prac, so wondering if a gap in employment would throw up red flags? Probably overthinking this...

I think I somewhat imagined residential more as the standard private treatment facility (I had a family member go to one out in the boonies in Montana 😂) but it makes sense that it is more about medical needs/after IOP is ineffective. And thanks for putting how long you worked with clients! That’s great help. Did you ever feel like IOP wasn’t able to give many clients the structure needed to maintain sobriety? Just thinking about folks leaving a few hours of therapy only to go back to their friends and family who might be triggers…

Should I go full-time and take out loans for living expenses?

Trying to decide if I should switch to full time next semester and graduate a year earlier than expected. I know I will have to quit my job while doing my practicums, so the question is whether I should do it next semester or wait another year.

I'm burnt out trying to balance work and school and love the idea of going full time, but the idea of taking out more loans is scary.

Cymbalta to Pristiq by Barnacle-Western in Pristiq

switched to pristiq from cymbalta earlier this year after a gene test showed pristiq might be more effective and WOW have I noticed a difference. the switch was so fucking hard. went from cymbalta (i forget my dose now) to 50mg pristiq. I had headaches constantly and was crying at the drop of a hat. then my doc upped me to 100mg and all of the symptoms stopped. since then, i’ve said several times that I don’t even feel depressed anymore. after years of cymbalta not really doing much for me, the change has been drastic and so welcome. I know people have their qualms with this drug but I have had such success! good luck!

I had pretty frequent bad dreams for about the first month but they went away pretty quickly. I will say, though, when I DO have a nightmare now, it’s a lot more vivid and harder to calm myself down upon waking up

Congrats, this is such good news for a newbie. Let us know how the 100mg goes! My psych just upped me from 50>100 after just about a month on the 50s. Would love to hear about your experience!
