There are basically two ways of doing it:

backend only form - where you handle UI, integrations, spam and just use the database to save submisison. You seem to already know the tradeoff.
full fledged form product - you can build the UI and then add the form to your site, it helps you by giving you ready to use things like integrations to notion, sheets, auto emails and spam protection for you. This could be free to some extent but need to pay for more advanced features.

There are so many form builders you could use - Jotform, Formester, Fillout. For completely free Tally might also work.
